Short-term relief
Planned breaks for caregivers and households, with familiar routines and supports kept at the center.
Transportation
Safe, timely rides
Transportation to work, Day Hab, appointments, and activities with clear schedules and documentation.
Care Coordination
Team planning
Coordination between households, case managers, MCOs, and other providers to keep plans aligned and follow-through consistent.
